Attention GPs working in Northern Adelaide (NALHN region)
Following on from the Adelaide PHN GP Round Table and CALHN GP/Hospital Consultant Workshops earlier in the year, where GPs identified a need for improving the relationship between acute and primary care, Adelaide PHN are working in partnership with the metropolitan Local Health Networks (LHNs) to hold workshops in each LHN area.
Adelaide PHN and NALHN now invite Northern GPs and Hospital Consultants to a Workshop on 16 October 2019.
Adelaide PHN and NALHN acknowledge previous engagement with GPs as part of the Northern Adelaide GP Liaison Unit. This workshop will provide further opportunity to meet hospital consultants, Adelaide PHN and NALHN Executives.
Using Enzyme methodology, this workshop aims to:
- facilitate meaningful engagement between hospital consultants and GPs
- identify issues and opportunities to facilitate improved communication and patient care
- identify ongoing forums to discuss clinical issues
- prioritise areas for action to improve the relationship between acute and primary care
